Return-to-play is where medicine, contract value, and impatience collide. The literature is clear that timelines are risk curves, not calendars — and the difference costs careers.
What this field actually measures
- Return-to-sport timeline (ACL)
Time from reconstruction to unrestricted competition.
ReferenceCommonly 9–12 months, with criteria-based clearance now favoured over time alone.
- Re-injury risk
Probability of a second injury after returning.
ReferenceYounger athletes returning to pivoting sport show substantially elevated second-ACL rates, and risk falls markedly with each month of delay past nine.
LimitRisk is a distribution. Individual clearance still needs individual testing.
- Limb symmetry index
Injured limb output as a percentage of the healthy limb.
Reference≥90% is the conventional clearance floor across hop and strength batteries.
LimitSymmetry can be achieved by the healthy limb getting worse.

Each month of delayed return up to nine months reduced reinjury rate, and passing return-to-sport criteria cut reinjury further.

- Which return-to-play test batteries actually predict re-injury rather than describe recovery?
- How should load history be weighted against tissue healing time?
- What is the long-term joint-health cost of accelerated timelines?
What we won’t say
We will not describe an athlete as 'ahead of schedule.' Schedules are marketing; tissue has its own opinion.
